We can provide medical certificates for a variety of reasons, including for:

• Illness or injury, where time off work is needed
• General health status certification for work/driving
• Medical fitness recreational activity such as PADI certificates for diving
• Renewal of ACC-related return to work certificates
• WINZ ( work and income ) forms

Please ensure you enter the reason for your medical certificate request when booking this service.

A standard medical consultation fee applies for this service, price can vary depending on the reason for the certificate and the assessment consultation time involved.

ACC-related certificates are $40.
